Our primary focus at North Colchester PCN is to improve patient care in new ways to promote independence and give you ownership of your own health. We have invested in different methods of support, from face to face consultations, to telephone support and mobile applications to track your health and wellbeing.

Below are a few examples of ways we can help you.

Physiotherapy

Our physiotherapist team are qualified autonomous clinical practitioners who can assess, diagnose, treat, and manage musculoskeletal problems and undifferentiated conditions. Physiotherapy appointments can be accessed directly by patients, speak with your surgery’s reception team to find out the next available appointment. Our team can establish a rapid and accurate diagnosis and management plan to streamline pathways of your care.

Health & Wellbeing Coaching

Our team use health coaching skills to support patients to develop the knowledge, skills, and confidence to become active participants in their care so that they can reach their own health and wellbeing goals. They also provide access to self-management education and peer support.

Mental Health Support

We have three clinical members of staff who provide different levels of mental health support across the PCN. They act as bridge between primary care and secondary mental health services and can facilitate onward referral to a range of services to meet patients’ needs. Please enquire with reception to learn more information and how to get an appointment.

Children & Young Persons Counselling

Our children & young persons counsellor can meet a wide range of needs, from early identification and intervention in primary care, to more targeted or intensive support and interventions as part of a joined-up approach with children and young people’s community mental health services.

Holly Health - Digital Wellbeing Support

North Colchester PCN have partnered with Holly Health, a personalised behaviour change platform for population health, self-management, tailored and guided education, and improvement of long-term health outcomes. Please contact your local surgery for access to the app.
