Maternity Support

North Colchester PCN have been working with local support services to improve the post-natal experience within General Practice for new Mums. We have compiled several resources below to support new Mums on their journey, in the hope that it highlights the support available locally. If you would like to learn more about these services or what else is on offer, ask your GP surgery to speak with your Social Prescriber.

6-8 Week Checklist

Once you have registered your baby with a GP surgery, you will be offered a 6-8 week check with you and your baby to check in with how you are doing. This appointment allows for conversation between you and the doctor about both you and your baby’s health. The following document is a resource you can use to support these conversations, and may prompt things to discuss with the doctor prior to the appointment. Please feel free to bring the document along with you to your 6-8 week check.

Signposting

We are working with Suffolk and North East Essex Maternity and Neonatal Services to increase awareness on support available outside of General Practice. This includes support with infant feeding, health visiting, maternity support, community support, and antenatal education. Click on the document to find out more.

Maternity Care Plan

Your Maternity Care Plan is a comprehensive document that allows you to keep note of your maternal journey, express your wishes, how you feel about certain parts of pregnancy, and offers support throughout. This document can be used for your own use, or can be used to share with any healthcare professional involved in your maternal care, and will help to express any wishes you would like to be considered throughout your pregnancy.
